INTRODUCTION

Le bruxisme du sommeil intrigue l'homme depuis fort longtemps comme en témoigne la Bible (Luc, 13-28 ; Mathieu, 13-42; Psaume, 102) qui décrit ces étranges manifestations nocturnes. Ces bruits insolites, provoqués par le grincement des dents, ne réveillent pas le bruxomane mais réveillent et inquiètent ses proches. Cette autodestruction est aussi considérée comme une punition puisque, lors du jugement dernier, il est question de pleurs et de grincements de dents.il est question de pleurs et de grincements de dents.

Le bruxisme peut se définir, d'abord du point de vue phénoménologique, comme des mouvements masticateurs et des grincements (ou/et serrement) des dents répétitifs et involontaires sans but fonctionnel (dit aussi para-fonctionnel), fréquemment inconscients, associés à l'usure anormale des dents et à l'inconfort des muscles de la mâchoire. Il doit donc être distingué d'autres types d'activité oro-faciale, telles que la mastication (à but fonctionnel), la toux, la déglutition, le bâillement ou encore le ronflement et la somniloquie. Il doit également être différencié d'autres types de troubles oro-faciaux, tels que la dyskinésie tardive, les tics ou le myoclonus.

Il est classique de distinguer le bruxisme nocturne et diurne, mais le terme bruxisme du sommeil est à préférer à celui de nocturne pour expliquer le fait que les gens puissent dormir et grincer des dents, soit le jour, soit la nuit. Ce bruxisme du sommeil est à différencier de celui observé durant l'éveil dénommé bruxisme de l'éveil. Ce dernier est souvent très limité dans le temps et correspond à des périodes de forte tension et de stress. Ainsi, le bruxisme implique non seulement la psychologie (stress, personnalité) et l'hypnologie par son interaction avec le sommeil, mais aussi l'odontologie avec ses conséquences oro-faciales, sans oublier les autres disciplines des neurosciences si on cherche à comprendre les mécanismes centraux impliqués dans sa genèse. Dans cette revue, nous nous intéresserons uniquement au bruxisme du sommeil.
